DEATH CASE

DOCUMENTS MAY REQUIRED

 

1.     Power of Attorney by his/her father /mother (parents)-Authority Letter to handle the case here on their behalf.

2.     Death Certificate(preferably translated into English)

3.     Medical Report (cause of death)- preferably translated into English

4.     Agency name/contact no etc , authorised to send the mortal remain to India

5.     Authority letter from the University authorising for insurance agency/transportation agency for the work, if any.

6.      Sanitary supervision & Quarantine Certificate-by Transporter

7.      Encoffining certificate with human- by Transporter

8.      Antiseptic certificate (Non-communicable disease certificate)- by Transporter

9.      Transit permit (permit for exportation of coffin)- by Transporter

10.  No objection certificate from Embassy (to be issued by Embassy)

11.  Certificate of Entry ofDeath (to be issued by Embassy)

12.  Passport dully cancelled(to be done by Embassy by Embassy)

13.  Letter issued from Embassy for necessary assistant on the way

14.  Certificate for disposal of mortal remain (at Indian Airport)

15.  Dead body clearance Certificate ( GOI-Airport Health organisation)

 

Note- All documents should be attested in original at Indian Embassy, Beijing. The Chinese documents, if any, should be translated/notarised (preferred) by the authority before its submission to Indian Embassy.

 

 

PLEASE TAKE HELPOF TRANSPORTER FOR ANY OTHER CERTIFICATES/DOCUMENTS REQUIRED

PROCEDURE/DOCUMENTS REQUIRED FOR DEATH CASE

 

1.     Get the contact details of the Kin(parent or family) in India and passport no of deceased.

2.     Inform the deceased parent & nearest Indian Embassy/Consulate with his/her passport no and company/university details. ( Embassy of India-010-85312559/2560 & 18612083609)

3.     The parent/family should be advised to nominate a person on their behalf to carry out all the formalities in China, if they are unable to come to the foreign country to take over the mortal remain.  In this case an AFFIDAVIT (in stamp paper) may be made in India and scanned copy may be faxed/mailed immediately to the authorised person.  The original copy may be courier in due course. Format as per Annex-A

4.     If required, get a Consent Letter from the parent that post-mortem of the body is not required and whatever reason given by the Chinese Authority that are acceptable to them.  Send a copy of the letter to the Embassy

5.     Authority from the parent - Authorise a person to collect the mortal remains at the Indian Airport on their behalf. A copy of the ID card (Passport, Aadharcard, Voter ID or any photo ID card) should be mail to authorised representative in China. Send a copy of the authority to the Embassy. Format as per Annex-B

6.     The Employer/University Authority/ Authorised person will inform the Insurance Company (if any). 

7.     The Employer/University Authority/Authorised will nominate a Transporter ( in consultation with Insurance company, if any). Send a copy to the Embassy

8.     All negotiation regarding transportation of the mortal remains to India, cost, date of transportation etc will be done by the authorised person.

9.     Get the FIR copy from the Public security Bureau.

10. Forward a copy of  FIR & passport (1st, last page & Visa page) to the Indian Embassy ( Mail Id- consular@indianembassy.org.cn & FAX No. 010-85312565/2515)

11. Get the Medical Certificate certifying the cause of Death from the authority. Forward  a copy to the Embassy.

12.  If possible and time, then translated/notarised the above two Chinese documents ( FIR & Medical Certificate) into English.

13.  Make list of belongings of the deceased to be send to his family in India. Declare Gold or costly items, if any.

14.  Insist the Transporter to complete the formalities of Embalming, Encoffining & Quarantine of the mortal remains as soon as possible. And get these THREE certificates from the Transporter in Original.

15.   Insist the Transporter to book an Air Ticket to India for the possible date of transportation. Get a copy of the Ticket or detail information ( date of transportation, Flight No, Time of Departure,Originating  & Destination Airport). Transporter has to provide Exit Permit for the body.

16.   Once the following ORIGINAL documents received, The Embassy of India will attest these documents for their acceptability in India.

a)     FIR

b)    Medical Certificate

c)     Original Passport

d)    Embalming Certificate

e)   Encoffining Certificate

f)      Quarantine Certificate

g)     Authority Letter by the parent

h)    List of belongings

i)       Air Way Bill (copy or detail information)

17.   The Embassy of India, Beijing will attest all the original documents and returned the same after CANCELLATION of the original passport.

18.   The Embassy will issue a DEATH CERTIFICATE on submission of FIR, Medical Certificate & the deceased’s original Passport and a report from the nearest Public Security Bureau in China.

19.   The Embassy will issue a Note Verbal (letter)to all concern for necessary assistance in CHINA and INDIA while transporting the mortal remains of the deceased to India. 

20.   The documents are to be send along with the body :- CANCELLED passport, Original Death Certificate issued by Indian Embassy, Medical certificate, List of belongings, Embalming, Encoffing &Quarantine  certificates. The belongings may be transported with  the body or in separate booking as per transporter or wishes of authorised person.

21.  The family members and the authorised person to receive the body at the Indian Airport particularly be informed about the arrival flight details as soon as possible.

 

Note-  1) All original dements are to be ATTESTED by the Embassy.  Submit a copy of each documents to the Embassy at the time of attestation.

     2)Any other documents other than above required by Transporter or Insurance company may be issued by the Embassy on request.

4.       If any consequence arises in future, the Embassy of India, Beijing, will not beheld responsible for that.
